This reminds me of the Excel scoring error by NASA that impacted where the Space Shuttles would be retired. Basically costing the National Museum of the U.S. Air Force an orbiter (though the NASA Administrator says it wouldn't have made a difference in the end). It was a bad enough screwup that Congress complained and NASA's OIG filed a report on the incident.
