I don't think he's saying "don't profile", more like "don't assume that all programs spend 80% of the time in 20% of the code". I know I've seen plenty of cases where the function at the top of the callgrind output is ~5% or less of total execution time.

Ye. I mean increasing the performance of semi-perpormant apps is really hard. It's tiny changes everywhere. At some point you got even the high hanging fruit and you are changing the order of if-clauses to decrease cache footprint to increase speed.
