Dedicated server provider is responsible for the hardware and the network. In this very important aspect, it's really no different than cloud (so yes, the price "includes" the repair).
It depends on the exact provider, but as a general rule, they are (a) slower to repair and (b) not as proactive (e.g. they might not detect an eminent failure). On even a medium-sized contract, you can generally work in 1-4 hour repair windows (many providers even offer this as standard), and better ones are definitely a bit more proactive.
There's like 1000 different dedicated provider, so it will depend on what yours offers, but unless it's a "managed dedicated provider", you're going to be responsible for your own backups. At best, you'll get some external storage you can use (e.g. Hetzner include free 100GB ftp you can use, if I recall).
