My company does yearly 360s. But you only can rate peers and maybe one level up. I think they could flush out incompetent managers (there are many) quickly if people could rank managers several layers up. I can think of several directors who basically destroyed the culture of their department. They were good at talking to their uppers but made horrible decisions in hiring and management. But there was no way to raise this issue in a safe manner.
