The roughly 150 combined military and civilian incidents that the two respective Wikipedia lists contain demonstrate that 1. military and civilian use of technology both go wrong roughly similarly often (approx. 55% and 45%, respectively) 2. in just about 100 years of "nuclear technology", there have been more than a mean of 1 1/2 incidents per year. I'd be surprised if the Wikipedia list is complete, there are probably substantial numbers of unreported/suppressed incidents, although I expect them to be small (large incidents would have been detected by others measuring radiation).
