After the bad thing happens — we look back and find that incidents occurred that were not handled. Eg they were bullying or bullied and complaints about the bullying were raised but were not dealt with. Preemptive monitoring not required. Or we find “concerns were raised” about behaviour, often many times, over many years, and the concerns were ignored. Anyone translating such things into a need for more pervasive monitoring is just pushing their own agenda.
