There's probably no one answer to the problem any more than there is to physical birth defects; sometimes it happens; you only have to worry when they start clustering. Also, if people are rewarded for exhibiting their abnormalities, then you'll find more people doing it, like the folks in poverty-stricken countries that pretend they have no legs for sympathy.
