FAA specifically should know better than blaming individuals. Whatever happened was a systemic issue because in a well designed system individuals simply should not be able to bring this much chaos. Neither accidentally nor maliciously. Is there anyone there who heard about just culture, and root cause analysis? I can’t even believe they would publish this irresponsible stuff.
