If the gov takes 1k in and another 1k goes to insurance companies, it kinda hard to get people to pay 1k(say a Dr offers a service outside both the gov and insurance) to do whatever when they have already paid 2k and gotten nothing. This is despite that just dealing directly with the dr is a 50% discount...
