Exactly 3 happened to me, but thankfully the provider just dropped the charge and I never had to pay. I had a nerve transplacement surgery in my elbow and wrist, and apparently some neurologist called into a video conference for ten minutes from the east coast and tried to charge $14,000 for that, and my insurance said no way. Honestly, I might have even consented to it, considering they gave me like 10 forms to sign as I was already in the gurney with an IV in my arm and the anesthesia drip had already started.
