If you look at the American Bar Association model rules they say "a lawyer shall abide by a client's decisions concerning the objectives of representation and, as required by Rule 1.4, shall consult with the client as to the means by which they are to be pursued. "
This means there are situations where the attorney does not have to follow the client's instructions. The client determines the objectives and how to plead, but this doesn't mean the attorney is obligated to listen to the client's order about everything.

Some other conflicting interests include duties to the court and legal professional privilege. Example client can tell you to do something that is against the interests of the administration of justice. when they insist you have to resign.
