It bugged me back then, too. IDK. In my job, just fixing an issue isn't enough. I want to know what exactly happened to cause the issue, why it happened, and what I can do to prevent similar issues in the future. I want to know how pervasive the issue is, if it's just for some users or all users, if it's getting better or worse. Seems like too much of how doctors are trained is "you're not paid to look any deeper than the bandaid."
