Sorry that you got down-voted for this. I tried to balance it out by voting it up. In general I think all of the possibilities that you mentioned with threads making new conversations etc. are true. For me, the issue is more in the UI/UX of both the Twitter website and the Twitter mobile applications and how threads are presented.
