I just heard an interview on ABC Radio with The Guardian journalist Anne Davies about safety issues during the construction of the Sydney Metro rail system here in Australia. Amongst other things a 30 tonne train carriage transporting equipment travelled 1.5km uncontrolled in a half built tunnel and an escalator being installed at an underground station fell 4 floors.
