We're supposed to "know", tut to ourselves, and move on. The combination of avoidance, passive writing style and implication, leaves the reader a perfect space to fill with their own prejudices. So now we can talk about massive public health decline in an offhand way, as if it were a force of nature. Obviously these poor and lazy people bring it upon themselves. At best they are victims of some kind of "economic determinism". Nowhere are the perpetrators in this picture.
