Weird as it is, urologists would be no stranger to erect penises. And why would they bring their wives to a boring medical lecture if they're not doctors? Sounds like some exaggeration for effect. I think it would have been more gasping or booing than screaming. This whole injection thing sounds highly uncomfortable by the way, no wonder it didn't really take off. At least they found a better way soon after..
