This study seemed pretty quick to attribute this to policy differences, but I didn't see any normalization for access to healthcare. Most Republican counties in Texas for instance are rural. At times, access to a major hospital can be hundreds of miles away. Specialists could be even further. In addition, the article mentioned Democrats are more likely to exercise. It seems more likely that the gyms/capita in urban counties would be much higher than the gyms/capita in rural counties.
