We're weird that way. Some insult words are fine, as long as you aren't talking about the actual body part. Although strangely, 'twat' is still considered by most people to be more vulgar than 'asshole', 'dick' or even 'dicksmack' (which I always thought was a hilarious term). AFAIK there is no equivalent 'twatsmack' but maybe there should be. Also nobody bats an eye when a Canadian talks about their favourite fanny pack.
