At a startup I worked at briefly (~4 weeks), engineers were constantly being asked to do ad-hoc pieces of work from people all over the business. A product manager who started around the same time as me tried to introduce a lightweight triage system for his team by which inbound requests would go via him. He was immediately taken to task by the CTO who told him to stop rocking the boat and not to introduce any other changes until the new VP of product joined in a few months.
Arguably he tried to change too much too fast, but anyway, there's a reason I only lasted 4 weeks.
