As someone who just got promoted into engineering management from development, the number one cause of frustration, delays, confusion was always lack of documentation - either in requirements or just how the system works in general. It’s now my number one priority to ensure that we have a proper framework for documenting requirements ie. no more two-line “change this functionality because reasons” tickets and having a centralized wiki that documents all features and how the work, along with any quirks that exist.
