A lot of complexity comes from premature scaling due to cargo cult or ergonomics. But I argue a lot of complexity and bugs comes from poor/unclear/conflicting thinking. Especially when it crosses boundaries between multiple developers who had to modify it but didn't truly internalize that part/design of the code.
