Incidents are mostly caused by untested configuration changes, infrastructure failures and often known limitations of software. A “hot take” could be something like code freezes are bad cause when it comes to most software and incidents the risk with changing the ways of working and not improving the software can be bigger than the risk with new releases.
I think it is ok with code freezes though.
