For me, camera off if possible, but don't mind turning it on if required. I also don't care if the other users have, and usually I prefer them off, but strangely I like it more when they have a profile picture, either a real life foto, an avatar or even an abstract drawing, something that allows me to "map" the sound to something while hearing it.
