I wouldn’t say so. It’s just a complex environment with many moving parts, not the least of which includes a last minute swap of type of aircraft. Ironically, it’s our human limitations which allows us to build systems which we can’t comprehend in entirety, but then our other positive human behaviors we use to identify problems (such as the passengers seated differently from the load sheet) which can save us.
