To be fair, American startup culture has many good characteristics. Independence, worker driven product development, some good management practices, at will employment - a good thing, keeps vested interests on their toes. Where America failed is healthcare being tied to jobs. At will employment must come with total freedom in life. Unfortunately a lot of workers did not jump ship often enough for management to feel the repercussions simply because they were afraid of consequences to healthcare. If healthcare were separated, many knowledge workers would simply quit the next day. They've got the money - they don't need to listen to a bumbling empire builder who only cares about themselves.
