I've worked at two tech companies with "unlimited PTO" and both have extremely strict reporting and request processes that prevent using it, even with months of notice. The worst instances were when PTO was approved well in advance, and then revoked too late to change plans, making it actually _cost_ me money.
The other two tech companies I've worked at had officially limited, roll-over PTO that accumulates with experience, and both had "honor system" reporting that allowed us to take more than that if we needed it for emergencies, as long as we kept people affected by it in the loop and made an effort to hand off any impacted work.
Naturally, the latter was the vastly better experience, and my unused PTO paid out to me when I left both jobs, unlike the unlimited PTO company.
