One theory I've seen advanced why the claimed reports to the NCMEC are many orders of magnitude larger than the prosecutions is that the conjecture that the whole thing is a CIA operation to collect kompromat on massive numbers of people. The alternative that the database is just stuffed full of non-illegal images seems more likely but I dunno if its that much more comforting.
