They explicitly don't want to implement it server-side due to the privacy concerns of decrypting every image in iCloud Photos. Doing it on-device limits Apple's possession of decrypted photos to those likely to be CSAM.

Apple can already decrypt your photos in iCloud - that’s how you can log in at iCloud.com and see them.

  Of course they can; The point is that they don't want to if they don't need to.
