The utilman.exe trick relies on having write access to the filesystem, something you cannot do while the machine is off (disk is encrypted) or at the login screen (system is live, you can't mount the disk). Mounting a disk from a hibernating system doesn't work either, in my experience. Of course, there are other ways to defeat Windows logon, but they're presumably more difficult.
