Hospitals operate 24x7 but individual Nurses and Doctors don't. They work in shifts of course! A 4 day working week means a hospital would have to hire more doctors and nurses of course. It's a thought bubble though: in my country doctors and nurses are severely overworked as it is. The health system could not permit 4 days a week for doctors and nurses.
