"Next page please!" "Flip over to continue reading!" and other insulting directions that more or less imply I don't know how to read a multi-page document. I haven't drooled in a while, but I do actually appreciate those kinds of hints. I agree with everything else you said though. It's hard to imagine that debasing the very concept of surveys will be a net positive for democracy.
