in the US the vaccination is already free, so no one should be paying for it As to the time spent, that seems unclear or not part of the survey though i know a few companies personally that are allowing employee's to use "on clock" time to go get the vaccine if they schedule it during the work day though when I got mine from check-in to out was less than 30mins, including the 10-15 mins they ask you to wait after getting the shot
