I sometimes suspect that this is a consequence of the anti-engineering mindset that a lot of the software industry embraces. Things like YAGNI, programmers don't need to study CS, not hiring testers, etc. Maybe if software "engineers" were treated more like actual engineers then we would start seeing better results.
