I find this part highly suspicious and it calls into question the motives of the "leaker" and wisdom of publishing this article. If someone wanted to discredit the vaccine finding some internal document around a process failure (even a temporary one), doctoring parts of it to make it more dramatic, and finding someone who will write about it is a good way to go about it. By the time the correction is published for the doctored material, or the matter is contextualized, the fear will have already spread.
