So, large, popular, commercial airplanes, are passing their mandatory and thorough pre-flight safe-to-fly inspections, but then having catastrophic failures within minutes of take off. It must mean that pre-flight inspections (to quote George Carlin) an illusion of safety? ie Not fit for purpose.
