I try to do everything in my power to keep two days entirely free for real work. If that doesn't work, at least I'll usually have one day without a meeting / distraction. Obviously, this is not always something you can control, but if you can I highly recommend it. If you are in a position to do make this decision, make it clear to everyone all the time that you're e.g. not available on Tuesdays.
