I'm sure a lot gets caught, but the amount that slips through is just unacceptable. Fortunately I haven't come across the kind of content you describe, but I reported ISIS propaganda on Twitter and was told it didn't break the rules. Shortly after complaining publicly and tagging @jack the tweet was removed. Could be a coincidence, I'll never know.
