  Not really. Ex-employee had access. If his password expired, he could just choose a new one and keep access. Revoking credentials of employees after they leave and doing regular audits of the access list, are controls that would be relevant here.

    I read it that wasn’t just his own account he was accessing but yes you’re not wrong about employee termination procedures also.
