  Sure, if you're diligent enough, there's nothing stopping you, there's no wealth barrier - studying medicine is a reasonable option for students from poor families. Is it not the case in USA? I have read about doctors coming out of med school with lots of debt, but I had presumed that these loans are reasonably available and would not prevent someone from a poor family from studying there.

  A doctor can be an entrepreneur too. After studying, you can opt to work in a clinic or start your own business. The latter is usually the faster path to wealth.

    Let's say that you are a homeless 40-yearold, where can you study and start a medical career?

      In Austria you could if you graduated from secondary education. But since you aren't born a 40-year old homeless person, I'll have to question that person's life choices and missed opportunities.
