I'm intrigued here. It clearly isn't the work of script-kiddie-type hackers, because of the research required and the subtlety of the phishing emails.
But equally, it does require some thorough-going technical competence – particularly to have got away with it for so long – so it's hard to believe that it's simply an unscrupulous insider/agent/scout within the publishing industry.
