On EC2, I've had a great experience using AWS Systems Manager [1]. I don't need any ports open, and it works great with normal shell tools and emacs given a .ssh/config like this:
host i-* mi-*
ProxyCommand sh -c "aws ssm start-session --target %h --document-name AWS-StartSSHSession --parameters 'portNumber=%p'"
