Even assuming they were well trained and safety minded, it's reasonable to assume they did not have the power to tell their bosses, "this is unsafe so we're not gonna do it." I am not going to reject outright the possibility of some nefarious plot, but corruption and bureaucratic incompetence explain all of the facts perfectly.
