I think that a lot of ideas here are really good, but they shouldn't be implemented in npm. Npm is not meant to be curated. NPM is meant to be a canonical way to reference a dependency, and that's about it. I don't think it's NPMs role to assign "quality" to a package (even though ironically they have a quality metric) -- that's something a third-party should do on top of npm. That's the unix philosophy in me speaking.
