This is the hypothetical everyone is supposed to agree with... Because the simple "COVID19 in the system when they die" is clearly not a proper definition. In practice: The big question is the consistency of the "heart attack" case. What each individual doctor determines to be a COVID19 death isn't really consistent in that situation. It can't be, every medical scenario is different.
