How do you manage your health data?

As an American much of my health data (ex. medical records) are on paper in a Doctors' office or in various closed systems. I'm curious if anyone has found a good solution for taking ownership of all your data and putting it in one place (ideally so it's still private but parts can be shared with relevant medical professionals on the fly).
