Years ago Merlin Mann, on his website, recommended Niel Fiore's book. I just reread it recently and it's a gold mine, for me anyway. My biggest takeaways were: 1. Recognize how I procrastinate ( more research, figure out the best place to start, another meeting, etc. ) and when 2. Chunk things: I can't build the whole project now, but I can work on feature x, y or z right now.
