Autovacuum can indeed ruin your day if you have a 24/7 DB that is delete or update heavy (the default setting are not very aggressive and once it falls behind badly it will never catch up and you'll need to do some emergency manual vacuuming). But I can assure you it's possible to run 24/7 DBs fine without running into autovacuming problems by making sure things are configured right. I've experienced both regular problems due to bad config and their complete disappearance.
