All of this rings true, with the exception of having some forms notarized. My name on my forms did not match the name on my credit card when I paid for my Apple Developer account. A short time later Apple deactivated my account and left it in that state until I could provide them with notarized proof of my identity. That is not terribly unusual, as far as I am concerned.
