As a pain specialist, I ask you to strongly incorporate one more element: Make it ridiculously easy for the patient to put your docs in touch with (other docs). At some point they will land in the hospital, and ONLY having your doc talk to the attending on the other hand will save them from under treatment and withdrawal.
