Fail fast, fail often doesn't work when your life is on the line. I'm sure Tesla did a good job testing but this is one of those things where the world simply is hard to work out what happened. Mind you that the self driving car is probably way better than a comparable human driver. Why do we not feel bad when a human accidentally kills himself from faulty programming too?
